Comprehensive Geriatric Care Program-CGC

Jewish Family Service provides affordable assistance that enables seniors to remain safe, healthy and independent in their homes to the fullest extent possible and as long as possible.

Services include:

  • Helping seniors create a safe environment in the comfort of one’s home
  • Personalized scheduling of Companions (2-24 hours from 1-7 days/week; hourly, live-in or sitter service in hospitals)
  • Companions are screened and professionally trained
  • Social Work supervision
  • Periodic (every two to three months) Home Visits and follow-up
  • Assessment and search for a Companion who best matches client’s needs and temperament
  • Temporary or long term care depending on need
  • Respite for immediate family caregivers

Home Companion Program

People who are afflicted with the following conditions can be assisted with the Home Companion Program: Dementia, Hearing and visually impaired, Alzheimer’s Disease, Parkinson’s Disease, Multiple Sclerosis, ALS, Heart Attack and Stroke. Companions can help promote recovery from surgery, illness or injury.

Services Provided:

  • Meal preparation and clean-up
  • Guidance with grooming and dressing
  • Assistance with bathing
  • Laundry and linen washing
  • Bed making
  • Exercising and socialization
  • Organizing activities in the home environment
  • Grocery and clothes shopping
  • Transportation to medical appointment, errands, social and recreational activities

Personal Care Services
Non-medical services provided include all the above as well as Certified Nurses Aides (CNA) assisting with feeding, bathing, dressing, toileting (changing undergarments), and tooth brushing and ileostomy care.

Geriatric Counseling

Licensed social workers offer individualized or group therapy, in-home when necessary, focusing on:

  • Managing at home
  • Dealing with change
  • Loss and bereavement
  • Depression and anxiety
  • Family issues
  • Isolation

Medicare and other health insurance accepted

Case Management

  • Offering assessments to identify problems, eligibility for assistance and need for additional services (such as in-home help, meal services and financial assistance)
  • Navigating the bureaucracy of Medicaid, Medicare, subsidized senior housing eligibility, etc.
  • Assisting with paying bills, transportation, and advocating for legal and financial matters
  • Assisting with moving to or from home, retirement complex or nursing home
  • Providing support for those with the responsibility of caring for an elderly family member e.g., checking on seniors for out-of-town relations and keeping them informed
  • Reviewing medical issues, social needs and home safety
  • Acting as a liaison between a broad range of senior and appropriate community services

Kosher food pantry
Provides free kosher food for those in need.
